Please Follow These Steps:

1) When filling out the "Empowerment Consultation Form" or when sending an E-mail, please include your name, address, contact phone (where you may be reached), E-mail address, if possible, and the best day and time you may be contacted.

2) You will be contacted via phone or E-mail to set up an appointment time. At that time, please make arrangements to send in your payment with a signed "Empowerment Consultation Form."

4) Upon clearance of your payment, you will be contacted for your one-hour "Empowerment Consultation" at your scheduled time.

PAYMENT
Phone Consultations: $40 (1 hour)
(Promotional Discount of $40 valid until Dec. 31, 2010.)
(Regular Price: $50)

1) A non-refundable appointment fee of $10.00 is included in the $40.00 fee. Full payment of $40.00 must be received at least one week prior to your scheduled session.

2) You may cancel your session up to three days before the scheduled session and receive a refund, less the $10.00 non-refundable appointment fee. You would be refunded $30.00.

3) At this time, only phone consultations are available based on schedule availability.

4) Once your appointment has been scheduled, you will need to send in your payment or pay with a credit card over the phone. You must also send in a signed "Empowerment Consultation Form" at least one week prior to your first phone consultation session. Thank you.
